- Method 1: Use the on-line web form.

- Method 2: Use your own word processor: Download a form in Word,
WordPerfect or ACSII text file format, fill out the form using
your word processor (you can save a copy for yourself) and submit it
as an attachment to an e-mail [instructions]. Send it
to: Records Committee Webmaster

- Method 3: Print a copy of the report form using Acrobat
Reader. Fill it out and send it to the address below.

| How to submit additional materials (photos, drawings,
etc.): |
- Method 1: If you can scan the photos or drawings and attach
the file to an e-mail, that is the quickest and easiest
method. (see
instructions)

additional materials by regular mail, please send it to the
